What You Will Do
As an Associate with Ascension WI, you will have the opportunity to in collaboration with a physician, provides for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.
Opportunity: Provide nurse practitioner family medicine services and operate the Racine Coalition Employer Based Clinic The Racine Coalition is a collaboration between Racine County, the City of Racine, and the Racine Unified School District Provide chronic disease management, occupational care, drug screening, primary care, sports physicals, annual wellness exams, medication refills, and medication management to patients ranging from 2 years old - geriatrics Hours may vary but will be 40 hours per week between 7:00 AM - 7:00 PM Monday through Fridays and 8:00 AM - 12:00 PM Saturdays
Responsibilities: Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening, immunizations and patient care. Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources. Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources. Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.
What You Will Need
Licenses/Certifications/Registration: Required Credential(s): BLS Provider credentialed from the American Heart Association (AHA) obtained within 2 Months (60 days) of hire date or job transfer date. One or more of the following:Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure is dependent on state associate works in.Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State. Licensure is dependent on state associate works in. Preferred Credential(s): Drug Enforcement Admin credentialed from the Drug Enforcement Administration.
Education: Diploma Master's degree of Nursing required
Work Experience: Previous experience in Occupational Health or Family Medicine preferred but not required
